Does Medicare cover me in Mexico?
No. Medicare does not cover care outside the United States, with very narrow exceptions. The day you land, you are paying for healthcare another way. Most people here budget US$2,000–4,000 a year in total at around age 60, in good health.
The detail
This is the most common surprise among American retirees here, and it arrives at the worst possible moment — usually in a waiting room.
Canadians face their own version: provincial health coverage generally stops applying once you are out of the province beyond a set number of months. Check your province's rule before you go.
What people actually do
Almost nobody buys one big policy. The common pattern is three parts stacked together:
- Pay directly for routine care. A private consultation here typically runs a fraction of what it costs at home. Many people simply pay cash for check-ups and minor issues.
- IMSS as a backstop. Mexican social security allows voluntary enrollment for legal residents, at roughly US$450–1,200 a year depending on age. The catch matters: pre-existing conditions can exclude you, and enrollment is not instant.
- Private major-medical cover for hospitalization and serious bills, from roughly US$2,000 a year and climbing.
Two things that catch people out
Travel insurance is not health insurance. It handles emergencies on a trip, not ongoing care — and it does not cover dental work, which is why so many people here pay for that in cash.
IMSS enrollment requires legal residency. If you come as a tourist for 180 days, that door is closed to you. That is one of the quiet reasons people move to temporary residency.
What to do now
- Call your own plan and ask, in plain words, what it pays outside the country. Get the answer in writing.
- Price all three layers before you decide. Many people over-insure because they priced only one.
- If you have a pre-existing condition, ask about it before you count on IMSS.
- Find out where you would actually go in an emergency here, and how you would pay on the spot. Knowing that in advance is worth more than most policies.
